Maternal Medical Nurse and Maternal Health Specialists are accessibly located at select OB/GYN offices; all of whom focus on patient-centered care, health risk assessment and reduction, psychosocial screenings, and patient education. PMH also serves as a “hub” by coordinating support, resources, and referrals for additional services.

Pregnant and postpartum women residing in Hillsborough County

• Services conducted at point-of-care during OB clinic visit or via web conferencing or telephone
• Prenatal risk screening and review
• Determination of eligibility for home visiting care coordination and evidence-based programs with expedited referral to program of choice
• Education on prenatal, interconceptional, and primary care including pregnancy-related, chronic disease, and illnesses
• Referrals and linkage to community resources, Medicaid, and other health insurance options
• Assistance with individual medical plan of cares and support to reduce barriers to care
• Screening and referral for intimate partner violence, substance misuse and misuse, and perinatal depression
